In chapter 9 you can have sex with him, but the encounter does not raise his relationship meter (only keeps it from regressing) so as of episode 9's ending you unfortunately cannot have a committed relationship established with Axel.
I thought I could play as female player character just like Eva's first game, but going into this it seems like I'm playing Ian.. is there a way to play as Lena for the whole game? I really can't relate playing as Ian at all :/

There are first three chapters of the game in the demo version. The current game version contains 9 chapters with next being soon to be released (somewhere in December).

First adult VN I've seen with genuinely well-crafted branching storytelling. I enjoy that you can make genuine choices in the sense that you're always giving up something to gain something else. A lot of the choices also have high stakes or long-term moral implications.
This is difficult to pull off, yet ORS does it repeatedly. They deserve serious props for the amount of interwoven cause and effect here.
My only gripe would be that I couldn't really warm up to the art of the sexual content. I can't quite place it but while the writing turned me on, the art didn't really complement it despite the obvious effort that had been put into it. Maybe I'm a bit spoiled by InCase's work but I think a little more detail in characters' expressions and facial details would go a long way.
Nevertheless, it's fun to play, seems to have actual replay value and a lot of the characters are very well written.
I think I know what the problem is. The sex scenes look like pornography, they do not look like people making love. For example, the picture of the two women almost kissing, tongues dangling like Gene Simmons. Who does that? Actors in porn movies.
